The $50 incentive applies to referrals for new patients outside your immediate family who book and attend an appointment.
Immediate family members (spouses, children, etc.) are not eligible for the referral incentive. Extended family such as brothers, uncles, aunts etc. are accepted.
The referred patient must provide your name at the time of booking to qualify.
Referring a family from the same address or medicare card will qualify for $50 and does not multiply by the number of people in that house hold.
Incentives are issued as gift cards or account credit and will be provided within 30 days of the referral’s completed appointment.
No limits or expiration on referral credits — spread the word to as many as you like!
